How to Register with the Surgery

To register with our surgery you must live within our practice area.

All new patients are required to fill in a health questionnaire giving details of medical history and lifestyle.  You will then be offered an appointment for a new patient medical check with a doctor.
 

PLEASE READ THIS IMPORTANT INFORMATION If you complete our online New Patient Questionnaire you will still need to attend the surgery in person to sign the necessary registration documentation.  Please bring with you 2 forms of identification with you. (This should include an item of photographic ID and a document showing your current address). Click here to complete our online New Patient Questionnaire (If you would prefer not to send your information via our online form you can collect a form from the surgery to complete by hand.)
